General Student Survey - DSPS 2007/2008

Instructions to complete student survey

 

      Thank you, for taking the time to complete the practice version of our survey. The practice version is identical to the actual version, except that when you submit the form, the content will not be sent anywhere. It is only for practice.


      The survey should only take a few minutes. You do not have to answer every question. Complete only those questions that you are comfortable answering. Some questions allow you to select multiple answers. Others will only allow one answer per question. Answer the text boxes as completely as you need.


      At the end of the survey, there is an area for you to provide contact information. This is only needed if you want us to contact you.
