Forms

HTML forms allow you to access shopping carts, search engines, Web-based email, bulletin boards, etc. Forms include controls such as edit fields, check boxes, radio buttons, combo boxes, and other controls similar to those used in dialog boxes.

  1. Edit field – An area on the web page where you can type in text.
  2.  Button – an area that can be clicked on in order to perform an action, such as submitting a form
  3.  Check box – a field that can be “checked” or “ unchecked” to indicate a yes or no decision.
  4.  Radio buttons – a group of check boxes tied together in one field.  Only one can be chosen in the group.
  5.  Combo box – A field that contains a list.   Only one item can be chosen on the list.

These are sample forms for practice only. Your submission will not be sent.
